It’s a fact! Children who are healthy do better in school!

To help us help your children do their best, a variety of health information is required on a yearly basis.  Here you will find the necessary forms and/or documents needed at enrollment and throughout the school year, policies and procedures as well as helpful information and web-sites on a variety of illness and health issues. 

If your child has any chronic health problem, food allergies or other health concerns, please contact the School Nurse.

Health Information for Registration

Kindergarten Students:  Before enrollment, take the following forms to your child’s doctor’s appointment:           

• Physical Examination Form
• Immunization Record
• Medication Form, if needed
• A list of question you may have.

All Students:  At Enrollment please bring the following completed forms, or the information needed to complete these forms during registration.

• Emergency Medical/ Activities Permission Form
• Immunization update or signed waiver
• Physical Exam Form
• Health History Form
• Request for Medication Administration OR
• Permission for Self-Administration Form

All Transfer/New Students at time of enrollment please bring:

• Current Immunization Record
• Physical Examination Form
• Any pertinent health records or information.
