To help us help your child, health information is required as well as current health updates.

At the time of enrollment, the following information/form will be required. See School Health Forms to download copies or for additional information.

Pre-School:

• Copy of Certified Birth Certificate
• Current Immunization Record
• Completed Physical
• Health History
• Permission for Medical Emergency/Activities

Kindergarten: If Student is new to the district or has not previously attended Blue Valley Pre-School, refer to "Pre-School list of required information. All others will need:           

• Updated Immunization Record
• Health History
• Permission for Medical Emergency/Activities

New/Transfer Students 1st-12th Grade:

• Copy of Certified Birth Certificate
• Current Immunization Record
• Physical Examination Form
• Health History
• Permission for Medical Emergency/Activities

All Returning Students Pre-K-12th Grade: 

• Updated/current immunization record
• Health History
• Permission for Medical Emergency/Activities
• Completed Physical Grades 6-12

For any Medications needed at school, any additional questions or concerns, please refer to School Health Forms, the Student Handbook, or contact the School Nurse.

Copies of all health forms are available at each school's office or can be downloaded from this website.
