Step 1: Select the enrollment packet that corresponds with the building your child(ren) will attend. If your child has previously attended USD 384, select the appropriate RETURNING STUDENT packet. If your child is new to the district, select the NEW STUDENT packet.
Documents can be printed and filled out at home, or they can be completed on your computer. If the PDF packet doesn't open right away, you will need to export them as a PDF once they have opened up in a new window. Just go to File > Export as PDF... > and save to your desired location. You can then open documents in Adobe Acrobat Reader (free download) or Preview (Mac). Adobe Acrobat Reader will be the best option for Windows computers, but can also be used on Mac. You will now be able to fill in the document on your device. Signatures can be added using the signature feature. (Adding a Signature pdf) Any documents that need to be signed by students can be left blank and we will have it signed at school.
Once finished, save the document to a desired location on your device. If you have more than one child to enroll, go ahead and select the corresponding packet and complete again.
*Note: There may be some forms that do not pertain to your child (Self/Medication Admin, 18yo forms, 4yo At-Risk, etc). Leave these pages blank when you submit the packet.
Physical forms need to be printed and completed at home. Forms need to be turned into the district office. Students cannot begin practicing or participating in PE until the physical is turned in.
*This form only needs to be completed by Pre-K or kindergarten students that are new to the district. If your child has already attended USD 384, this form does not need to be completed. This includes students that attending PreK and are moving onto kindergarten.
Step 2: Submit completed documents. Completed documents may be mailed or delivered to the district office in the middle school. Packets can also be left in the drop box by the front doors of the middle school.
For an online option, enrollment packets may be submitted through our Google Form. Follow the link to the form, answer the two questions, and attach all enrollment packets at the bottom.
Step 3: After your enrollment packet have been reviewed, you will receive an email notification with your enrollment fee totals and a request for any missing information.
